POSITION PURPOSE
– To assist the Child & Family Health Nurses in the establishment of a culturally
appropriate service to specifically target Aboriginal children and their parents in the
0 – 5 age range
– To positively engage Aboriginal communities guided by Primary Health and Early
Intervention frameworks and strategies
– To work in collaboratively within the multi-disciplinary healthcare team to achieve
better outcomes for Aboriginal children, families and communities
– To work in partnership with the Child & Family Health Nurse in collaboration with the
local Aboriginal community
– To build capacity within existing mainstream clinical services in order to provide
improved access to care for this cohort of the Aboriginal population.
RELATIONSHIPS AND REPORTING LINES

The Family Support Worker will work collaboratively all other team members of AWAHS.

The Family Support Worker will be expected to liaise with key stakeholders who are involved with health issues relating to the local Aboriginal community. The liaison will also include other local Aboriginal Organisations.
The Role and its Responsibilities
• Work with the Albury Wodonga Aboriginal Community to implement the Action Plan agreed to by NSW Health (Building Stronger Foundations)
• Work in partnership with Aboriginal families, Child and Family Health Nurse and other
AWAHS staff to promote infant and child physical, emotional and social health
through assessment, consultation, case management, group work, education,
supportive counselling and referral as required
• Assist, develop and facilitate community consultation processes with Aboriginal
families and communities
• Support the multi-disciplinary team in providing culturally appropriate care to
Aboriginal children, women and their families
• Work in a multidisciplinary team in conjunction with members of the AWAHS team,
Child and Family Health Services in NSW and Victoria etc to deliver integrated care
for the families of this service and in building relationships between Aboriginal
organisations and mainstream services
• Develop linkages and partnerships with key service providers including; allied health
professionals, playgroups, schools, community health centres, hospitals, councils,
government and non-government agencies that will promote culturally appropriate
referral pathways for families
• Assist in the development of culturally appropriate health promotional strategies to
promote better health outcomes for infants, children and families eg; breastfeeding,
smoking cessation, with particular emphasis on strengthening community action and
building healthy environments
• Assist Aboriginal families in their access to the Immunisation programs and facilitate
access to follow-up immunisation services for children identified by the Australian
Childhood Immunisation Register as being overdue for immunisation
• Provide a joint home visiting service to all parents and their families on at least one
occasion with the C&FHN and provide ongoing home visiting to families with
complex needs consistent with the NSW Health Supporting Families Early Policy
• Assist families in attending appointments and engaging in services to which referrals
have been made or recommended
• Assist in collating, recording and collecting service data and in compiling service
reports
• Support and plan for programs including playgroups, baby groups, women’s groups
and parenting educational programs
• Adhere to the requirements of the Child Wellbeing Unit and Child protection policies
and procedures for NSW health including the requirement to be a mandatory
reporter of child protection concerns
• Attend and support multidisciplinary case reviews and case planning meetings for
families and children with identified vulnerabilities and concerns
• Develop and maintain relevant professional skills and knowledge, consistent with the role, through education, research and practice and adhere to AWAHS policies, procedures and guidelines
• Demonstrate a commitment to quality improvement and participate in team quality activities
• Undertake mandatory training/education and other duties as directed by management in accordance with award requirements
• Participate in regular supervision and peer support meetings.

• Bachelor of Health Science, Graduate Certificate or Diploma in Health or Aboriginal
Health Studies Certificate 4 or working towards same or related qualifications and/or
experience in Aboriginal Health
• Experience in working with families who have young children
• Active involvement in the Albury Wodonga community and commitment to
improving the health of Aboriginal and Torres Strait Islander peoples and experience
working with Aboriginal and or Torres Strait Islander organisations

The Role and its Responsibilities
The Aboriginal Disability Liaison Officer will:
• provide outreach to Aboriginal and Torres Strait Islander communities in a culturally
sensitive manner, breaking down barriers to accessing the NDIS and developing trust
and rapport
• link potential Participants to their PITC and/or customer, including through online
platforms and resources
• support potential participants and their representatives to attend appointments to
get functional assessments and other evidence for access requests
• linking potential Participants to their PITC or customer to help complete Access
Request Forms (ARF), Verbal Access Request (VAR) and psychosocial disabilities
evidence forms
• supporting potential participants in obtaining updates with respect to submitted
forms from the relevant PITC and/or customer
• linking Aboriginal and Torres Strait Islander people who are not eligible for NOIS to
community mainstream supports and/or refer them to their PITC to support linkages
to the community and mainstream supports
• supporting participant handover to PITC and/or NOIA by:
– assisting participants or potential Participants build a positive relationship with
local PITC and/or the NOIA;
– supporting local PITC and/or the NOIA to understand the needs and
circumstances of participants or potential participants
– working together with PITC and/or the NOIA to develop trust and rapport with
the Participants or potential Participants; and
– supporting Aboriginal and Torres Strait Islander Participants and their
representatives to link positively with PITC and/or the NOIA.
The ADLO also undertake collaboration activities including:
• working collaboratively with already existing NOIA community engagement
activities, including by not limited to:

during planning and review meetings (if required) by providing non-biased support
• supporting Aboriginal and Torres Strait Islander Participants and their representatives
to connect with NDIS Complaints and/or NDIS Participant Critical Incident processes
(if required); and
• assisting Aboriginal and Torres Strait Islander participants and their representatives
with submitting a request for a plan review (if without a support coordinator).
General Tasks and Overview
• Establish and maintain effective relationships with participants and their families,
carers and advocates, NDIA, other mainstream and disability service providers,
members of the community, networking groups, funding bodies, government
agencies, auditors and practitioners
• Provide quality support based on developing skills, competencies and resources to
enable the participant and their family to promote the participant’s wellbeing and
development and the family’s capacity to be self-managing

Health Promotion and Awareness
• Provide culturally appropriate health care to meet the needs of the community
• Participate in relevant local community projects and groups that aim to increase general awareness regarding the effects of nutrition and diabetes health issues
Clinical Services
• Education to clients on procedures and treatments to gain cooperation and
understanding, and to allow clients to make informed decisions about their health management
• Safely and proficiently undertake a range of clinical assessments, tests and
procedures as a member of a primary health care team, interpret clinical findings using a problem-based approach based on a working knowledge of organ systems and common diseases, determine an action plan, use written care protocols and liaise with senior health staff

The Role and its Responsibilities
The MCH Nurse working in the Victorian Aboriginal Maternal and Child Health program will focus on:
• Universal Health Home Visit to families with a new baby in Wodonga and some
outlying areas
• Providing Key Age and Stage (KAS) visits (0–5 years) as per the green book and
Victorian MCH Practice Framework and Guidelines
• Provide additional visits including home visits to families requiring additional support
• Work alongside an Aboriginal Family Support Worker within the program, offering additional supports to families
• Providing parenting support strategies and activities which may include group work
such as parenting groups

• Building capacity within existing mainstream clinical services to provide improved access to care for this cohort of the Aboriginal population

Black Summer Grants program YACVic’s ‘Future Proof: Young People, Disaster Recovery and (Re)building Communities’ Project (Future Proof) is funded by the Australian Government’s Black Summer Bushfire Recovery Grants Program, which provides funding for projects centred on community recovery and resilience activities in response to the 2019-20 bushfires.
The project commenced in 2022 and must be completed by June 2024. Youth Affairs Council Victoria (YACVic) is the lead coordinating partner for Future Proof, bringing together a coalition of fifteen organisations from fire-affected communities in Eastern Victoria.. Future Proof project partners receive funding to employ local youth worker and young peer worker positions to support place-based, youth-led activities, projects and community decision making focused on bushfire recovery and resilience. The coalition will be made up of community organisations, local councils, Aboriginal Community Controlled Organisations (ACCO’s), and local youth organisations.
Additionally, the project includes qualification pathways for 150 local young people to kickstart careers in emergency management and community services, the creation of local youth advisory groups, training for young people and youth workers, and a community of practice for workers to support and learn from each other.

Position Scope
The Youth Engagement Officer will be responsible for engaging First Nations youth input and participation into disaster recovery work. The role will guide youth participation and engagement activities including facilitating input to the overall project direction by local young people through the project reference group and developing engaging local projects and activities to aid disaster recovery work from the 2019/2020 black summer bushfires.
This position will support the Youth Resilience Coordinator in the Coalition Partnership of the Future Proof Project including attending the regular Community of Practice Meetings with the Youth Resilience Peer Workers and coordinating training for local young people in collaboration with project lead YACvic. This position will work alongside other partners to assist local young people in accessing and undertaking qualifications and supporting young people to take on traineeships and jobs in line with their qualifications. This role will work with partners to ensure all project deliverables are implemented in line with contractual obligations. The role will actively promote opportunities to young people and local community and
recovery focused organisations. The role will provide high level guidance to local
community and key partners and stakeholders regarding best practice in youth
participation, ethical practice and engagement on disaster recovery and preparedness. The role will actively participate in developing input to evaluation and research activities alongside partners.
Major duties
– Proactively engage and support youth work content in line with the contract
obligations for Future Proof, engaging with Coalition Partner representatives and
stakeholders.
– Co-develop, deliver, and implement the project plan according to agreed timeliness, including monitoring and reporting on project milestones.
– In collaboration with participants, co design and develop project deliverables including consultation workshops, workshop materials, frameworks, and youth participation activities.
– Implement effective communication and engagement strategies with internal and external stakeholders including young people, expert advisors, emergency
management sector organisations and organisations working with young people.
– Support and encourage young people’s input into project activities including the project youth reference group.
– Participate in activities as part of the Future Proof coalition partnership including

– Demonstrated knowledge and understanding of local Aboriginal Community and an ability to communicate effectively and sensitively.
– Demonstrated experience in the rural youth/community development sector and an understanding of the issues important to rural and regional communities, particularly as they relate to young people, in fire and disaster affected areas.
– Demonstrated experience in Community & Youth Engagement projects aimed at young people (12-24).
– Understanding of youth participation principles, ethical practice principles in youth work and how they are enacted in community settings.

Position Scope
This position is available to people aged 25 years or under. The Youth Resilience Peer Worker will assist in the delivery of youth led local bushfire recovery activities as part of the Future Proof Project. The role will assist in developing AWAHS youth participation and engagement activities in North East Victoria / Southern NSW to aid disaster recovery work from the 2019/2020 bushfire.
This role will assist with coordinating local training activities targeting young people. This role works with key stakeholders to develop local activities that focus on including young people in bushfire preparedness, recovery, and resilience.
As part of the role, you will participate in a Community of Practice (COP) which brings all fifteen partners together for training, project development and support related to youth participation, and ethical youth work practice in bushfire recovery projects.
The role will encourage young people’s voices to be heard and for them to take an active role in community bushfire recovery activities. The role will actively support and promote opportunities for young people and relevant community organisations including supporting participation on local youth advisory structures, providing input to resources, evaluation tools and research activities alongside other project partners.
You will be supported to obtain or upgrade your qualifications as part of this role and you will support other local young people to undertake qualifications, seek out and take up traineeships locally. The position is supervised by the Youth Resilience Worker.
Major duties
– Assist with the strength-based design, planning of and facilitation, of online and
face-to-face meetings, consultations and training workshops alongside local young
people.
– Support local Future Proof Coalition Partner organisations, especially those with peer workers, to deliver project outcomes for young people.
– Provide input and support the development of program materials and events ensuring they are effective for the relevant youth audience.
– Support young people to participate safely and effectively in meetings, consultations training workshops and activities, and help them to understand their rights and responsibilities.
– Participate in the Community of Practice alongside other coalition partners (and relevant guests) including specific peer worker meetings, gatherings, and training.
– Assist with planning, logistics, and delivery of both large and small project gatherings for young people related to the project.
– Assist in recruiting and supporting young people to successfully undertake qualifications as part of the project and to participate in traineeship opportunities.
– Ensure professional standards are upheld, including co-design principles, Child Safe
Standards and the framework of the Code of Ethical Practice in Youth Work. (You will receive training in these.)
– In partnership with Victoria University, actively contribute and support young
people’s input and contribution, to research and evaluation activities including focus groups articles and publications.
– Contribute to and provide support to the Social Emotional Wellbeing team in
reporting on project deliverables and ensure compliance with all funding
requirements.
– Attend and actively take part in regular staff meetings and activities.
– Carry out any other duties your supervisor or manager may assign to you, having
regard to your skills, training, and experience.
– Travel across Victoria will be required for this role including overnight trips, and
regular travel across AWAHS geographical catchment
– This role will require weekend and youth friendly hours, and the successful candidate
will need to be able to travel independently to events.
Eligibility Criteria and Requirements
Essential
– Experience in design and facilitation of consultations and training workshops or other projects for young people. For example, coaching a sports team or running school leadership workshops.
– Demonstrated understanding of appropriate behaviours when working with diverse
young people.
– Understanding of the issues important to rural and regional communities, particularly as they relate to young people, in fire and disaster affected areas.
– Demonstrated ability to develop positive, collaborative working relationships with a
range of stakeholders, strong relationships with peers and others such colleagues,
teachers or community leaders.
– Willingness to undertake qualifications in youth or community services as part of the
role.
– Great organisation and administration skills, with the ability to prioritise tasks.
– Willingness to undergo a police check.

Desirable
– Experience in working with communities impacted by natural disaster will be an
advantage.
– Lived or learned experience of youth cohorts who often face marginalisation
(including but not limited to, rural and regional, disabled, Aboriginal and or Torres Strait Islander, LGBTIQA+, multicultural, OOH care and youth justice young people).
– Demonstrated experience working confidently with Microsoft programs – preferably including Office 365.
– A current Victorian Drivers Licence would be an advantage but is not essential. However willingness to obtain a Drivers Licence is preferred.
