Transport Driver – Albury

The transport driver is directly responsible for ensuring that AWAHS patients/ clients are safely transported to and from their medical/ specialist appointment on time. The role promotes the services of AWAHS in the community and that the services relative to the role are undertaken in a manner that enhances the professional image of the organisation.

Medical Administration Receptionist

Aboriginal Identified Position

Suitable applicants will need to be able to provide high quality culturally competent reception and administrative services to patients, doctors, and staff at AWAHS. Ensuring that patients appointments are managed in a professional and timely manner that meets or exceeds their expectations.

General Practitioner | Albury

Part Time- 3 day per week

The General Practitioner role is a 3 day a week position that is involved in closing the gap for the local Aboriginal and Torres Strait Islander community. The GP will be directly responsible for providing primary healthcare services and associated referrals to AWAHS Clients.

Drug and Alcohol Support Worker | Albury

You will be directly responsible for the provision of case management support to clients with drug and alcohol dependencies. Working within the Social Emotional Wellbeing team and working with other AWAHS teams you will be providing a holistic level of care. This position promotes a client focused approach to engaging with clients affected by alcohol and other drug use.

ITC – Chronic Disease Outreach Worker | Albury

Full Time | Aboriginal Identified Position

The role of the CDOW is to assist in reducing chronic disease morbidity and mortality through investment in preventative social marketing activities, expansion of the Aboriginal health sector and building the capacity for health care services to deliver effective services to manage chronic disease for Aboriginal and Torres Strait Islander people.

The CDOW will work closely with the Integrated Team Care (ITC), Care Coordinator as the role of the Care coordinator is to direct the work flow to the CDOW. The CDOW will also work as part of the overall ITC team and service providers and networks to build capacity to offer Aboriginal and Torres Strait Islander patient services in the local community. The position will also work to support services to improve transition between components of the service system for the Aboriginal and Torres Strait Islander community.

Chronic Disease Outreach Worker | Albury

Full Time 9 month contract | Aboriginal Identified Position

The role of the Chronic Disease Outreach Worker (CDOW) is to assist in reducing chronic disease morbidity and mortality through investment in preventative social marketing activities, expansion of the Indigenous health sector and building the capacity for health care services to deliver effective services to manage chronic disease for Aboriginal and Torres Strait Islander people, including smoking cessation and remote patient iHealth monitoring. 

The CDOW will work closely with the chronic disease nurse, chronic disease team, and the Integrated Team Care (ITC) chronic disease outreach worker. The CDOW will also work with service providers and networks to build capacity to offer Aboriginal and Torres Strait Islander services in the local community. The position will also work to support services to improve transition between components of the service system for the Aboriginal and Torres Strait Islander community.
